Referral form to Pure Land Home Hospice

The services provided by Pure Land Home Hospice free, generalist in nature,
holistic, and are designed to support you and/or your loved one to stay at home as
long as you wish, living the best quality of life possible. We provide holistic care,
helping meet your social, emotional, spiritual and clinical needs.

We work with other agencies, including GPs, allied health professionals, specialist
palliative services, aged care providers, and disability services to help you live the
best quality of life you can.

After receiving a successful referral, a Pure Land Home Hospice staff member will contact you for a further conversation about your palliative care needs.

Have a progressive life-limiting condition; and

Want to stay at home as long as possible; and

Live within the local council areas of

Understand that we cannot provide 24/7 care and therefore you have a carer who can be there when needed?
